All children develop at different rates, but around the age of 2 or 3 years toddlers often begin to show a growing desire to start to become independent. 

This is one of the most important times for social and emotional development and can be both exciting and challenging for all the family.

Health Visitors work with parents and carers, offering support from the ante-natal period until the child starts school at five years.

The serviceincludes a team of Health Visitors, School Nurses, Staff Nurses, Nursery Nurses and Parent Support workers.  They work with families to help support children’s health and development, to make sure they have the best possible start in life.

You as parents/carers know your child better than anyone. The service will work in partnership with you to promote your child’s health, development and well-being from the antenatal period, right through the Early Years.

Parents and carers will continue to have unlimited access to the service for advice and support via the Hub, the ‘Chat Health’ text service and local Child Health Clinics.

In addition, you will receive an appointment for your child’s 2 - 2½ year growth and development review.

This appointment will be to review your child’s growth and physical, social and emotional development, to offer support and advice and answer any questions you may have. 

The assessment is completed in partnership with you as a parent /carer with either a Health Visitor, Specialist Public Health Practitioner or a qualified Nursery Nurse.

This is an opportunity to ask any questions or share any concerns that you may have.

Support is available from the service until the young person reaches 19 years of age and can offer advice and guidance on many topics. This might include parent’s mental health, feeding, sleep, behaviour, potty training, child development or emotional wellbeing.

If your child attends an Early Years setting they will monitor your child’s communication development as part of the Early Years Foundation Stage framework. The setting will talk to you about your childs development putting in place activities to support their development.

The two year integrated review provides a progress check covering a childs health and development before their third birthday.

This review gives parents the information to support their child to be ready for school. It should also make sure that any children who may need a little extra support and help get it early.

What does the integrated Review involve?

Progress check – if your child is attending an early years setting they will complete a Progress check when your child is aged between two and three. Parents and carers will receive a copy of this check

The Progress Check should be taken to your childs Growth and Development review with the Health Visitor service/ Families Health and Wellbeing Service.

The Health Visiting Team covering Stoke on Trent/ Staffordshire Families and Wellbeing 0-19  service will send an appointment for a Growth and Development review to all Children aged between 27 and 30 months. This is a parent/carer questionnaire, to assess the development of a child. The review will be completed by a Health Visitor, Specialist Public Health Practitioner or a Nursery Nurse.

If the child is already inan Early Years Setting the review will be integrated with the Early Years Progress Check completed by Nursery/Child minder at the age of 2.

The review will include anassessment of the child’s growth and development to identify any additional need in order that further support or early intervention/advice/referral to specialist agencies, can be made and the service offered tailored to the family’s need.

Your local Family Hub is a virtual space where you can find advice, support and connect with others in your area.

The Family Hubs offer a range of information, support and advice for families at a local and county level. This is communicated mainly through the Family Hub Facebook pages but can also be answered by individual queries by email, message or phone calls. 

We also have produced a wealth of in-house support which has been created by the Early Years Coordination team.
