This award will be given to the individual who demonstrates how compassionate and professional they must be to be able to support someone with dignity who is at the end of their life.  They should be able to provide evidence that the learning and development they have acquired has been consistently put into practice when supporting someone at the end of their life.  
How have they supported family, friends, have they devised and followed an advanced care plan, how have they ensured the person is comfortable and as pain free and dignified as possible?
