Financial assessment for care services
If you do not complete this form within 14 days of your care services starting you will be required to pay the full cost of your care services from the date you started receiving care, plus any administration fees applicable. Due to the current demand for completing financial assessments; if you have not received a Care Assessment, and you will not be receiving care in the near future, then your request for a financial assessment will not be treated as a priority.
Adult Social Care is not a free service. We will complete a financial assessment which will consider your savings, income and outgoings to calculate how much you will need to pay towards the cost of your care.
You must provide us with these details by completing a Financial Assessment form.

You must support your application with evidence, details of what evidence is required can be found in the form. This includes bank statements for the last six months, benefit award letters, proof of pensions, and details of your rent and or council tax payments.
