Your feedback is crucial in shaping our understanding of adult social care services, ensuring they meet good quality standards and are responsive to peoples care needs.
What is a Quality Experience Form?
The forms aim to capture your experiences and provide us with a broader understanding of care quality. They will help us identify areas of strength and areas that may require improvement. Each form covers a broad range of questions, designed to gather an understanding of your views of a care service. Each feedback question is optional, enabling you to only provide feedback on areas with which you have experience.
The form utilises a mixture of rating-based questions and comments to ensure the form is easy to complete but also ensures that you can provide additional narrative.
How will we use the information?
We will use the information that you share with us to better understand care services and inform quality assurance processes.
If you choose to share your name and contact details with us, they will not be shared outside of Staffordshire County Council. We may contact you to better understand your submitted form.
Ratings and comments might be shared with care providers or referenced in publicly available documents. This information will be presented to preserve the anonymity of the people who have completed a form.
What should I do if I have a compliment or issue that I need to raise?
If you need to raise a compliment or a focused quality concern that you’ve been unable to resolve, you should utilise the Quality Assurance Form (QAF) process. If you think someone who has care and support needs is being abused or you think their safety is at risk, you should utilise the Safeguarding process.
