The 'one form'

Blue and yellow sections

  • Updated Send and inclusion request form​

  • The areas in yellow are family support related questions, only complete this section if family support is needed.​

  • Once the form is complete. Email the form to the central inbox as you currently do sendinclusionsouthstaffs.staffordshire.gov.uk

​Grey section

  • This is where the recommendations from the hub are recorded. ​

  • If this is the first time the child or young person has been discussed, you will need to action the advice and recommendations.​

Green section

  • If the recommendation from the Hub is to request EAPDR support, you will then complete the EAPDR request form which is coloured Green.​

  • You only complete this part if you are advised to apply for EAPDR.​

Orange section

  • If the panel agree EAPDR is required, you move on to the orange part/EAPDR process of the form. ​

  • This is to be completed during or after the first assess and plan meeting takes place. ​

  • This is a co-produced meeting led by school and with all professionals/ EP and parents/carers taking part. ​

  • You will use the information gathered in the meeting to update the form.​

The 'do' phase

  • It is important to note that during the Enhance ‘do’ phase, you must update the form bi-weekly with a record of how the interventions/provision is taking place. ​

  • It will help form the enhanced review part of the process.​

  • These updates all then feed into the review meeting. This is where everyone comes back together to discuss the outcomes of the ‘do’ phase. ​

The review meeting

  • This is a decision point and collectively you will decide if another cycle of EAPDR is needed.​

  • If an ECHNA is decided, this will happen in the same way as it does now. ​

  • The form allows all evidence and reports to be uploaded, so all evidence is in one place.​

  • Should an ECHNA be needed the beauty of the form is that all the evidence gathering has taken place. In turn this will lead to plans being richer in content and ready in a timely manner. ​
