Advice and guidance

Annex C requests should only be made by Staffordshire, Warwickshire, West Mercia and West Midlands and only when the alleged victim is or was aged 17 years or younger at the time of the offence being investigated.

Please ensure you make your request as clear as possible and provide as much information as you can e.g. previous names, addresses, aliases.  Wherever possible provide a date of birth as this will ensure we provide information on the right person if it is a common name.

Ensure you state which statutory power you are requesting the information under.

Please note: we do not accept Schedule 2: Part 1 (Paragraph 2) of the Data Protection Act 2018 as a statutory power.

It is important that your request is countersigned by someone of inspector rank or above. We will accept an email from an inspector or above, attaching the form, if you do not have an inspector at your station when the form needs signing.

However, if you do send in an incorrectly countersigned form, it will be returned to you and will delay us in providing any information we may hold.
