Make an Annex C request

Annex C requests relate to the following forces only:

  • Staffordshire Police
  • Warwickshire Police
  • West Mercia Police
  • West Midlands Police

and

  • CPS West Midlands

Annex C requests relate to an allegation of physical or sexual abuse by a victim aged 17 years or under either currently or at the time of the offence.

Please note: you should complete and return either the Word or PDF version of the form.

These should be emailed to accessinformation@staffordshire.gov.uk and marked for the attention of Ruth Simmons.
